Returning Family with 1 Child

This is a non-refundable fee to reserve your child's spot in the appropriate classes as we plan our 2023-2024 season. After registration closes, you will receive your child's class placement, schedule, and instructions on how to enroll and pay for classes.

Waiver

I understand that STEP assumes no responsibility for personal injury or loss or damage to my Child’s personal property. I agree to release and waive liability for all claims that I or my Child may have, or may in the future have, against STEP, or any person(s) associated in any way with STEP, from any and all liability for any loss, damage, injury or expense that my Child may have suffered as a result of his/her participation or presence at STEP, due to any cause whatsoever.

Photo/Image Release Consent

Photographs or video recordings of my Child’s image may be taken during STEP activities. My Child will not be named in their publication. I hereby grant STEP the right to use all or a portion of the images in all forms of media including advertising and related promotion throughout the world and in perpetuity.

Declaration

I declare that I, by submitting this form, am legally authorized to submit this Consent/Waiver From and hereby give my full consent for my Child to participate in the activities and conditions cited above. I have read, understood, and agree to thecontents of this Consent/Waiver Form in its entirety and I sign it freely and voluntarily without any inducement.
